Community action agency for Andover, Ashford, Bolton, Brooklyn, Canterbury, Chaplin, Columbia, Coventry, Eastford, Ellington, Hampton, Hebron, Killingly, Mansfield, Plainfield, Pomfret, Putnam, Scotland, Somers, Stafford, Sterling, Thompson, TElegibility:HPRP provides funding to assist families and individuals who are at imminent risk of homelessness. Must be "imminently at risk of becoming literally homeless within the next 6-15 days":*Being evicted within 6-15 days from private dwelling (own home, or family/friend s home), OR*Being discharged from an institution in which the person has been a resident for more than 30 consecutive days, OR*Living in housing that has been condemned by housing officials and is no longer considered habitable, OR*Fleeing a domestic violence housing situation. IN ADDITION TO ONE OF THE ABOVE:*No appropriate housing options have been identified, and*The person lacks the financial resources and support networks needed to obtain immediate housing or remain in their existing housing.NOTE: If literally homeless or imminently homelessness (within 5 days), refer to homeless shelter. Resident of Tolland County or Windham County with a non-recurring, unexpected, documentable hardship; For financial assistance applicant must have received a notice to quit; Must have income; Income must be at or below 60% of the state median income; Monthly rent/mortgage cannot exceed 60% of gross monthly income; Must be able to document a severe hardship which is not chronic or recurring; Landlord must be willing to cooperate with mediation process; Must be able to pay the next month s rent; Cannot apply more than once in an 18 month period; Other restrictions may apply Hours of operation:M: 8am-6pm; T,W,F: 8am-4pm; Th: 8am-4pm Application must be filed within six months of leaving TFA; Client obtains a referral letter from their DSS worker; Client fills out the pre-application form on the other side of the referral letter; Client then mails the completed application to John D Amelia and Associates.
