Supportive housing agency provides scattered site housing and comprehensive supportive services, including case management, permanency planning, crisis intervention, counseling, substance abuse treatment, recovery supports, linkage to community resources, independent living skills instruction, transportation, and advocacy, for people living with HIV/AIDS, mental illness, substance addiction, and dual diagnosis.Elegibility:HPRP provides funding to assist families and individuals who are at imminent risk of homelessness. Must be "imminently at risk of becoming literally homeless within the next 6-15 days":*Being evicted within 6-15 days from private dwelling (own Hours of operation:M-F: 9am-5pm; Staff on call 24 hr/7 days Call to request a pre-application form; Must complete and return by mail or in person the completed pre-application form before an interview can be scheduled; Cannot apply more than once in any 18 month period
